Reminder: ALL campers must submit a Health Assessment Form and signed by a physician annually. ANY Health Assessment Form that is dated
August 2023 - current will be accepted.

This "Providence Summer Camp Parent Packet 2024" contains an overview of the camp procedures, policies, as well as the paperwork to fill out and hand in. Be sure to read this important information.
The following pages need to be filled in completely and accurately in order for your child to attend camp:
- Cover/Signature Page (Please print and sign)
- About Me (Please complete electronically)
- Child Health Report (To be completed by physician)
- Emergency Contact/Parental Consent Form (Please complete electronically)
- Agreement Form (Please complete electronically)- Technology Release (Please complete electronically)
- Video/Photo Release (Please complete electronically)
- Medication Log - if applicable. (Please complete electronically)Your child cannot attend camp without handing in the appropriate paperwork on or before their first day of summer camp.
